2026 Signature Verification Form

All annuitants are required by the LABF to submit an original, notarized signature verification form each year. The 2026 Signature Verification Forms were mailed on April 20, 2026. The forms must be signed by the annuitant, or their legal agent on record with the LABF, in the presence of a notary. 

If you are receiving a monthly annuity, please be reminded that the completed form must be returned to the LABF by May 31st to avoid interruption of annuity benefit payments. If you have moved, need a replacement form or have any questions, please call 312-236-2065.
