The LABF requires that its annuitants submit an original signature verification form each year.  For those who have yet to submit their form, a second mailing of the 2026 Annual Signature Verification Forms will be mailed June 10th.  The form must be signed by the annuitant, or his or her legal agent, in the presence of a notary and be notarized.

 

Please be reminded that the completed original form must be returned to the LABF by July 31st to avoid interruption of annuity benefits.
