Healthcare Subsidy Eligibility

Please note, the deadline to apply for the healthcare subsidy for the period 1/1/2017 – 12/31/2019 is December 31, 2020.

Pursuant to the Circuit Court of Cook County’s order, the LABF notified all annuitants who were eligible to receive the health insurance premium subsidy of $25 per month for all or a portion of the period between January 1, 2017 and December 31, 2019.

The deadline to submit a completed Health Insurance Subsidy Eligibility Form and Sworn Statement (“Form”) for this period is December 31, 2020.

To be eligible:

  1. Annuitant must have retired on or after August 23, 1989;
  2. Annuitant must have been hired prior to June 30, 2003;
  3. Annuitant must have at least 15 years of City of Chicago service;
  4. Annuitant must be 65 years old or older; and
  5. Annuitant must have either:
  • participated in a group healthcare plan for which the LABF deducted health insurance premiums from a monthly annuity (currently the Blue Cross Blue Shield plans sponsored by the City of Chicago or the Aetna plans sponsored by the Labor Benefits Association)*

OR

  • participated in any health insurance plan and paid their healthcare insurance coverage themselves, either through an account on which the annuitant is named or an account established for the benefit of the annuitant.

*Employee annuitants who fulfilled the eligibility criteria 1. through 5.a. during the time-period of 1/1/2017 through 12/31/2019 were already paid for the time-periods that they participated in the group plans indicated in 5.a. above.

The LABF is unable to process any subsidy payment until it has received and accepted the completed and signed Form. Please refer to the notices for details. Sample copies of the notices may be viewed at: www.LABFChicago.org/SubsidyNotices.
