2019 Annual Signature Verification Form

The LABF requires that its annuitants submit an annual signature verification form. The 2019 Annual Signature Verification Forms were mailed on September 23, 2019. The forms must be signed by the annuitant, or his or her legal agent and notarized. If you are an annuitant, please be reminded that the completed form must be returned to the LABF by October 18, 2019 to avoid interruption of annuity benefits. If you need a replacement form or if you have any questions, contact the LABF.

2016 marks the end of a three-year phase out of City of Chicago (“City”) retiree healthcare plan coverage.
