2021 Signature Verification Form

The LABF requires that its annuitants submit a signature verification form. The 2021 Signature Verification Forms were mailed on June 10, 2021. The forms must be signed by the annuitant, or his or her legal agent, in the presence of a notary and notarized. If you are an annuitant, please be reminded that the completed form must be returned to the LABF by July 15, 2021 to avoid interruption of annuity benefits. If you need a replacement form or if you have any questions, please contact LABF.

LABF’s Annual Participant Statements were mailed. Annual Participant Statements are only mailed to members who have not retired from the LABF. If you have not received your statement or wish to update your marital status or beneficiaries on file, please call 312-236-2065 to speak with a Benefits Counselor.
