2024 Signature Verification Form

The LABF requires that its annuitants submit a signature verification form. The 2024 Signature Verification Forms were mailed on April 24, 2024. The forms must be signed by the annuitant, or his or her legal agent, in the presence of a notary and notarized. If you are an annuitant, please be reminded that the completed form must be returned to the LABF by May 31st to avoid interruption of annuity benefits. If you need a replacement form or if you have any questions, please call 312-236-2065.​

Annual Participant Statements

LABF’s Annual Participant Statements were mailed in April. Annual Participant Statements are only mailed to members who have not retired from the LABF. If you have not received your statement or wish to update your marital status or beneficiaries on file, please call 312-236-2065 to speak with a Benefits Counselor.

Trustee Election Certification

Notice is hereby given that the trustee election which was to be held on Thursday, October 10, 2019, has concluded because the incumbent candidate, Michael LoVerde, ran unopposed.
